Fantastic Four

Congratulations to four Beacon Medical Group sites for being recognized nationally for outstanding patient centered care. The National Committee for Quality Assurance honored the following locations for outstanding achievement in providing team-based, coordinated, comprehensive and accessible care:

• Central Neighborhood Health Center
• Centennial Neighborhood Health Center
• Portage Road
• E. Blair Warner

This accolade highlights the goal of the four sites to reduce hospitalizations, lower medical costs and most importantly, enhance quality of life. As patient-centered medical homes, the medical staffs deliver care in the right place, at the right time and in the way that best meets a patient’s needs, including wellness, prevention, acute care and chronic care.

Care teams meet patients where they are, helping them address the most simple to the most complex conditions, in a way that fosters trust and respect. In addition, patients feel “right at home” because they are educated and supported by compassionate, expert medical professionals on an ongoing basis to make decisions and participate in their own care.
