Homecare

Homecare services are designed to help recovering, disabled, or chronically ill individuals stay in their home environment. These people don’t require all of the support found in a hospital or a nursing home, but they typically need medical treatment and/or assistance with daily living activities. The care we provide is tailored to the needs of each individual after a careful evaluation of his/her situation.

Hospice

Hospice is a special way of caring for a person whose disease cannot be cured. Through our hospice program, we provide services to patients who have a terminal illness and a life expectancy of six months or less. Hospice emphasizes pain management, symptom control, emotional support, and enhancing the quality of life. Using a family-centered approach, we provide assistance and support to help patients and families remain together in the home with peace and comfort.

Insurance Coverage

Our fully accredited homecare and hospice services are certified by both Medicare (traditional and HMO) and Medicaid (traditional and HMO), we participate with Blue Cross/Blue Shield and many other commercial plans. Our office can help you determine your insurance coverage if you or a loved one needs any of these services.

Three Rivers Health’s homecare and hospice services accept patients regardless of race, color, religion, national origin, sex, age, or disability, and will not deny admission to people with contagious diseases — including, but not limited to, TB, HIV, MRSA, and Hepatitis B — unless otherwise indicated by the State Health Department.
