Digital Registration and Check-In
Before your appointment
When you schedule your appointment, please provide your mobile phone number. In the week leading up to your appointment, we’ll ask you to confirm that appointment and complete your registration online for a faster, easier check-in when you arrive for your appointment.
- Complete the digital (online) registration from the comfort of home―or anywhere!
- You can register online prior to your visit or even when you arrive the day of your appointment.
After you arrive
When you arrive at the office you can check in without visiting the front desk. Here’s how:
- Look for the orange sign with the QR code when you arrive at your provider’s office.
- Scan the QR code using your mobile phone’s camera.
- The system will check that your registration is complete. If needed, you’ll be prompted to finish the registration process.
- Have a seat in the waiting room. If our staff has any questions, they’ll call you to the desk.
