Required copays, coinsurance or deductible amounts will be collected in advance of appointments for some medical services. Out-of-pocket payment amounts due are determined by patients' health plans. Patients who have Medicaid coverage or who are prequalified for financial assistance will not have an out-of-pocket amount due.

Although every effort is made to ensure estimates are as accurate as possible, there will be instances where the patient will owe Beacon an amount that differs from the amount presented on the estimate.
Patients who overpay at the time of service will be issued a refund once their claim is finalized and paid through their insurance plan. The timing of this refund will vary depending on how quickly the claim finalizes through the patient’s insurance company and could be issued anywhere from several weeks to several months after the patient’s visit.
If the patient owes Beacon for a separate, previous service, any overpayment amount will be applied to that service before a refund is issued.

Once your deductible is met, you pay a fixed amount called a co-payment, usually at time of service. This amount varies by plan and is set by your health insurance plan. Typically, if you have a lower monthly premium, you will have a higher co-payment.
Coinsurance is the portion you pay for a covered health care service, calculated as a percentage. Let’s say your insurance plan allows for an office visit that is charged at $100. Your coinsurance percentage is 20%, which means you pay $20. In addition, you would also pay any deductible you have remaining. If you have not met your deductible, you would pay the full $100.
A deductible is the amount you are responsible to pay before your insurance will begin paying for services. After this deductible is met, you will typically only pay co-payments and coinsurance. For family plans, there is normally an individual deductible for each person covered by the plan and a family deductible, which applies to everyone. That means, in a family of four, if one person meets his or her individual deductible, it applies to the family deductible, bringing that total down. If two other family members meet their deductibles and the family number goes to zero, all four individuals are considered to have meet their deductibles.
Providers who contract with your health insurance plan or company are considered “in-network.” Co-payment through in-network service providers is typically less costly than out-of-network providers. (See “co-payment” for definition.)

Your network of health providers, suppliers and facilities have contracted with your health insurer or plan to provide you services.

There is a limit to how much you pay out-of-pocket for health care in one calendar year. After you pay this total, your health plan will pay 100% of your covered benefits. Some health insurance or plans will not count all of your co-payments, deductibles, coinsurance payments, out-of-network payments or other expenses toward this limit.
Before you get a service or fill a prescription, you may be required to have approval from your health insurer or plan. Preauthorization is not a promise your health insurance or plan will cover the cost.
